Did you know that you can submit your child’s school and sports forms through the web portal? Scan your forms, attach them to a message in your child’s web portal account and select “WV DOCUMENT REQUEST” to receive the message. We will have the forms filled out and sent back you within 24-48 hours. If you’re not able to scan the forms, you can drop them off at our office and we will send them back securely through the web portal. There will be times when your child might not have an up to date physical and an appointment will have to be scheduled for your child to be seen before we can provide you with these forms.
