New Patients & Newborns

    We’d like to welcome you as a new patient to our practice.  Choosing a pediatrician is a very important step in your child’s healthcare.  We offer a meet-and-greet visit during which you can tour our office, meet our team, and consult one-on-one with our physician regarding any questions you might have.
    We are affiliated with Winchester Hospital.  If you are delivering at another hospital, the house physician will let you know when to book your appointment.  As always, we are just a phone call away. 

New Patients

Things to do before your first visit:

  • Confirm that our office has a contract with your current insurance carrier.   (View our list of contracted insurances
  • Call your insurance carrier and change your child’s primary care physician to one of our providers.
  • Request your child’s Medical & Immunization Records from your previous physician. You can use our Records Release From Previous Practice. Fill out this form and fax it to your previous practice to request your child’s records.
  • Read our Notice of Privacy Practices Form.
  • Remember to bring your child’s insurance card so that we can scan it for our records.
  • Fill all new patient forms prior to your visit.  
  • Once we receive a copy of your records and these forms are completed, we will contact you to schedule an appointment.

New patient forms are  labeled N1-N4.  

Newborns

Things to do before your first visit:

  • Confirm that our office has a contract with your current insurance carrier.   (View our list of contracted insurances
  • Call your insurance carrier to add your baby to the policy.  Some insurances only give 30 days to enroll your newborn. 
  • Choose Reading Pediatric Associates and one of our physicians as your child’s PCP.
  • Make the insurance coverage, as well as the PCP, retroactive to your child’s date of birth. 
  • Once your child is issued his/her insurance card, bring the card in for us to scan for our records. 
  • Read our Notice of Privacy Practices Form.
  • Fill all new patient forms prior to your visit.  

New patient forms are  labeled N1-N4.  

Your newborn child's next appointments:

These appointments should be scheduled at the end of each visit.

  • 2-3 Day Follow Up – Follow up from hospital and billed as a sick visit not a well visit. 
  • 2 Weeks   –  Well Visit
  • 1 Month   –  Well Visit
  • 2 Months –  Well Visit
    Vaccines Administered:  Dtap, IPV, Hib, PCV20, Hepatitis B,  Rotavirus
  • 4 Months –  Well Visit 
     Vaccines Administered: Dtap, IPV, Hib, PCV20, Hepatitis B, Rotavirus
  • 6 Months –  Well Visit
      Vaccines Administered: Dtap, IPV, Hib, PCV20,  Hepatitis B, Rotavirus
  • 9 Months –  Well Visit
  • 12 Months – Well Visit
    Vaccines Administered: Hepatitis A, MMR, Varicella
  • 15 Months – Well Visit
    Vaccines Administered: Dtap, IPV, Hib, PCV20
  • 18 Months – Well Visit
    Vaccines Administered: Hep A
  • 24 Months – Well Visit 
  • 30 Months – Well Visit
